The TempCare Transition Program is designed to support patients being discharged from hospitals and rehabilitation facilities who lack family or friends to assist them. This program ensures a smooth transition by providing essential care packages, temporary home care assistance, transportation services, and referrals to long-term support services. Our goal is to reduce hospital readmissions, improve patient well-being, and bridge the gap in post-discharge care.
Day Services
We offer multiple tiers of essential care packages tailored to support recently discharged patients with professional caregiver assistance during recovery.
1-Day Care Package - $99 (8 Hours of Care)
- Provides essential personal care, including hygiene, dressing, and mobility assistance.
- Ensures proper nutrition and medication reminders for overall well-being.
- Offers companionship and emotional support to reduce isolation.
- Includes light housekeeping and transportation assistance for post-hospital discharge or follow-up appointments.
3-Day Care Package - $275 (24 Hours of Care)
- Includes all services from the 1-Day Package, with additional errand assistance for prescriptions and supplies.
- Provides meal planning and preparation to support a balanced diet.
- Enhances post-discharge support by ensuring smooth home adjustments and follow-ups.
1-Week Care Package - $625 (56 Hours of Care)
- Expands on the 3-Day Package with comprehensive household support, including laundry, grocery shopping, and home safety checks.
- Keeps families informed with regular updates on their loved one’s well-being.
- Incorporates health monitoring and wellness support, such as mobility exercises and condition observation.
24 Hour Care Services Provided
1-Day (24-Hour) Care Package - $500
- Provides round-the-clock personal care, including hygiene, mobility support, and repositioning for comfort.
- Ensures proper nutrition with full meal preparation, feeding assistance, and hydration support.
- Offers medication reminders, overnight monitoring, and emotional companionship for reassurance.
- Includes light housekeeping, laundry, transportation assistance, and regular family updates on recovery progress.
3-Day (72-Hour) Care Package - $1,500
- Extends all services from the 1-Day Package for continued recovery support.
- Enhances home safety monitoring to prevent falls and ensure mobility.
- Expands meal planning and hydration support with tailored nutrition for healing.
- Adds daily health monitoring and errand assistance, including prescription pickups and grocery shopping.
1-Week (7-Day, 168-Hour) Care Package - $3,500
- Builds upon the 3-Day Package with structured mobility exercises for rehabilitation.
- Provides full household support, including laundry, cleaning, and home organization.
- Manages medical appointments, including scheduling follow-ups and therapy sessions.
- Equips family caregivers with training and transition support for continued care at home.
